Subsequently, the cause of engine failure was determined
to be a break in the linkage of the fuel controller system. So all
aircraft with this type fuel system were grounded for inspection
to be sure the same thing would not happen to some other guys.
If we had the gangstart modification, it would
never have happened. We had the paperwork, but no parts. If we had
cut through that higher headquarters feedback two weeks sooner like,
"the exigencies of the situation preclude the economical feasible
modification," which is bureaucrateeze for "I don't understand
it, I don't want to make a decision, and I'm waiting for retirement,"
the pilot's five kids would still have a father.
So when you're dealing with flight
safety items, you are really in a race against time
in something that is really a matter of life and death.
